1. I declare the information above is accurate and, upon issuance of any ORRA licence, I agree to abide by and observe the applicable rules as outlined in the ORRA Rules & Regulations document and any other applicable regulations under penalty of Suspension or Revocation at ORRA’s sole discretion.

3. I agree to be re-submit a current medical declaration as follows:
   • Upon the expiration of my current medical as required by the current competition rules
   • Following any significant illness, injury or hospitalization.
4. I give permission to any hospital, institution or physician to furnish my medical information to ORRA

If an applicant does not meet the required criteria for their desired license, the application will be declined, and any fees paid will be refunded..
